Answer: Most likely that's the case. I clean mine about once each week or so. Here's how I do it...concluding you are using the mouse like the new versions for the Mac and/or IIgs; (A) take the bottom circular plat off by unscrewing it. (B) take out the ball. (C) Get a bright light and a magnifying glass to look at the little plastic rings around the inside, using a pair of tweezers or the like, gently remove the crud on these plastic circles until they look clean. (D) blow the insides out with your breath. (E) using either isopropyl Alcohol or some such mild solvent, dip a Q-tip in it then swab each of the plastic circles until they look clean and shiney. (F) put the ball back in the mouse and screw the circular cap back on. Try it after all those steps and it should be working fine again. If not, then you've got other problems that might require fixing of a broken connection, loose wire, etc. in which case I'm at a loss to give any more advise.
